Help assess strategies for asthma control in California

Will you take a few minutes to share that expertise to improve asthma control in California?

The California Department of Public Health’s state asthma program, California Breathing, is working with partners – including RAMP – to develop a Strategic Plan for Asthma Control for California. We need your input.

RAMP is sending this survey to clinical providers, health care organizations, local health departments, managed care organizations, community-based organizations and others to assess the current strategies for asthma control in your geographic area. The survey will take approximately 15 minutes of your time.

Results from this assessment will inform strategies to improve asthma control across California communities. If you work on asthma in California, please take a moment to participate.

California Breathing will share the results of this assessment with you once it’s complete. Thank you in advance for participating! Access the survey here.
